Care Well supports the belief that persons with developmental and other disabilities require a strong multifaceted approach to the habilitative services being offered.

Residential Support

Provides assistance with the acquisition, retention, or improvement in skills related to activities of daily living, such as personal grooming and cleanliness, bed making and household chores, eating and the preparation of food, and the social and adaptive skills needed to enable individual to reside at home.

Community Networking

Provides individualized day activities that support the individual's definition of a meaningful day in an integrated community setting, with persons who are not disabled.  

Community Living and Support

Provides habilitation and skill building to enable the individual to acquire and maintain skills, which supports more independence. Community Living and Support augments the family and natural supports of the individual and consists of an array of services that are required to maintain and assist the individual to live in community settings.

Day Support

Provides assistance to the individual with acquisition, retention, or improvement, self help, socialization and adaptive skills. Day supports are furnished in a non-residential setting, separate from the home of facility where the individual resides. Day Supports focus on enabling the individual to attain or maintain his or her maximum functional level and is coordinated with any physical, occupational, or speech therapies listed in the Individual Support Plan

Respite Services

Provides periodic support and relief to the primary caregiver(s) from the responsibility and stress of caring for the individual. This service enables the primary caregiver to meet or participate in planned or emergency events, and to have planned time for him/her and/or family members. Respite may include in and out-of home services, inclusive of overnight, weekend care emergency care (family emergency based, not to include out of home crisis).

Supported Employment

This service supports a beneficiary to explore, and make an informed choice about pursuing, competitive integrated employment. Supported Employment-Individual services provide assistance, based on individual circumstances, and need, for an Innovations beneficiary to explore, seek, choose, acquire, maintain, increase, and advance in competitive integrated employment.
